New Patient Registration
We are pleased to announce that we accept new patients who live anywhere , including outside of Cambridge under the new Out of Area Registrations policy. All UK residents have the right within the NHS to be registered with a doctor. We are delighted to accept new patients onto our list.
If you wish to register with the practice, please visit us and talk to our receptionists, or print off the registration form below and bring it in to us.
It would be a great help if you could bring with you your up-to-date medical card. If you cannot find it, don't worry.
In order to register you we will need the following information:
- Full name, including previous names
- Date and place of birth
- Last address
- New address and post code
- Last name and address of your previous doctor
- NHS number if known - not the same as your National Insurance number!
If you have never been registered in the UK before you will not have an NHS number. In which case we will need the date you first arrived in this country, and details of your current address and any previous UK addresses.
It is important to realise that if you register with us but do not live locally, we will be unlikely to be able to visit if you deem that necessary. In these occasions we recommend that you call 111 who will advise you of the most appropriate route of care for your needs. Please remember, however that if you feel that you have a medical emergency, you can always call 999 for assistance.
You must print off a Registration form, Mandatory Patient Questionnaire and the Sharing of Information form, fill them out and bring them with you to your first visit to the practice.
